A woman who was hospitalized following a head-on collision during a police chase has died.

Officials say 25 year-old Rafalea Trevino died after doctors took her off of life support. A 6 year-old girl and 7 year-old boy remain hospitalized and two other individuals died in the crash. Police say the accident was caused by a 19 year-old Mission man who was eluding police after breaking into a home. He started a police chase and drove the wrong way down a road and hitting the other vehicle head-on. The man was also hospitalized in serious condition.
